Please be informed that applications for Certification of Travel Records are only accepted and processed at the head office of the Bureau of Immigration, Intramuros, Manila for reasons of security and uniformity.
You must present and submit the following:
a) Accomplished application form; and
b) A photocopy of your passport biopage.
If you are unable to personally process your application, you can designate a representative by providing the appropriate Special Power of Attorney (SPA), apostilled if you are outside the country, and a photocopy of your representative's government issued ID.
You or your representative will file the application, along with the aforementioned requirements at window 23 of the ground floor of the head office. Application fee is Php 1,040 with a processing time of 7 working days or Php 540 with a processing time of 15 working days.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to OIC Commissioner Joel Anthony M. Viado. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
