identified by the Ministry of Agrarian Reform (MAR) as an Applicant-

Settler and lawful occupant of Farm Lot No.[REDACTED], Survey No. [REDACTED],

located at Barangay [REDACTED].

We hold a Certification dated April 6, 1984, issued by the then

Ministry of Agrarian Reform, confirming his status as occupant and

applicant-settler for the said lot. The record is also referenced in

later project inventories of the Metropolitan Waterworks and

Sewerage System (MWSS) for the Laiban Dam Project.

Pursuant to Executive Order No. 2, s. 2016 (Freedom

of Information), I respectfully request:

1. Verification of the existence and current status of the 1984 MAR

record for my late father [REDACTED], Farm Lot No. [REDACTED], Survey No. [REDACTED], [REDACTED];

2. Certified true copy or scanned copy of the said 1984 certification

and any related MAR/DAR file entries (e.g., application forms,

resettlement or land-tenure records, lot plans, or award registers);

and

3. Official guidance or list of requirements for the legal heirs

to obtain a successor certification or certified true copy for estate

documentation and coordination with MWSS and DENR.

If the record is archived under another DAR office or agency (e.g.,

Central Records Division or Land Tenure Improvement Program),

please endorse this request accordingly under Section 6 of E.O. No. 2.

This is to respectfully correct a typographical error in my recently

submitted FOI request titled:

“Verification and Certification of 1984 MAR Record (Applicant-Settler,

Farm Lot No. [REDACTED], [REDACTED]).”

The correct lot number is Farm Lot No. [REDACTED], Survey No. [REDACTED], Barangay [REDACTED] — as indicated in the original MWSS

and Ministry of Agrarian Reform records of my late father, [REDACTED].

Please note that all other details in the request remain accurate. I apologize for the oversight and kindly ask that the search and verification be made under the corrected lot number [REDACTED].
