I am pleased to inform you that your FOI request has been approved. Please be informed that the encoding/updating of vaccination records is done by the concerned vaccination center. If you want, you may kindly coordinate with the Local Government Unit (LGU) where you received your vaccination and request for assistance to generate your vaccination certificate. For further assistance, you may call the Vax Cert Customer Service at (02) 8876-1488 or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
