Davao Occidental General Hospital (DOGH) respectfully submits its reply to the memorandum issued on August 11, 2025, requesting a response to eFOI Request #DOH-922898708102 & #DOH-703730294348. Attached herewith is the document related to Aggregated Data on Terminally Ill Patients in Magsaysay, Davao del Sur, for the years 2020-2025.
Please be advised that we also referred this FOI request to the DOH Regional Office 11 Davao and DOH Hospitals such as Davao Regional Medical Center and Southern Philippines Medical Center.
