We have received your request on Correction of vaccination card and certificate, dated Aug. 19, 2024.
While our primary objective is to provide information whenever possible, we regret to inform you that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate.

Your right to request a review
If you are dissatisfied with our response to your FOl request, you have the option to request an internal review.
