I am pleased to inform you that your FOI request has been approved. About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.

Your right to request a review
If you are dissatisfied with our response to your FOI request, you have the option to request an internal review.
