Hello, For a research project on the Integrated Tuberculosis Information System (ITIS), I am requesting: 1. A list of all entry fields for a patient's account when first enrolled and for any subsequent visits (in case those are different); and 2. A list of all types of information that can be part of a patient's account. (Ex.: name, date of first admission, diagnosis, app logins) Note that I am not asking for the disclosure of patient information. I am only asking for types of information, not the patient information itself. (My previous request for this was misinterpreted and denied, and I was not given an address to request an internal review: https://www.foi.gov.ph/agencies/doh/integrated-tuberculosis-information-system-itis-data-and-entry-fields/)
