I am pleased to inform you that your FOI request has been approved.

Magandang araw!

Mangyaring ipagbigay-alam na maaari ninyong ma-access ang inyong VaxCert sa pamamagitan ng pag-download ng eGov PH Application.

Narito ang mga hakbang kung paano ito gawin:

  1. I-download ang App – I-install ang eGov App sa inyong smartphone o tablet sa pamamagitan ng App Store o Google Play Store.
  2. Magrehistro ng Account – Ilagay ang inyong mobile number at kumpirmahin gamit ang One-Time Pin (OTP).
  3. Patunayan ang Iyong Pagkakakilanlan – Ilagay ang inyong pangunahing impormasyon at kumpletuhin ang facial recognition process.

Kapag nakarehistro na, maaari nang mag-log in at piliin ang serbisyo: HEALTH, pagkatapos ay piliin ang VaxCertPH.
Kung ang inyong vaccination record ay hindi lumalabas, maaaring hindi pa ito naiu-upload sa Vaccination Information Management System (VIMS) ng inyong vaccination center.

Kung nais ninyo, maaari kayong makipag-ugnayan sa inyong Lokal na Pamahalaan (LGU) kung saan kayo nabakunahan upang humingi ng tulong sa pag-generate ng inyong vaccination certificate.

Para sa karagdagang tulong, maaari kayong tumawag sa National Patient Navigation and Referral Center (NPNRC) sa numerong (02) 8651-7800 local 2431-2435.

Your right to request a review
