Greetings! I am writing this to formally request and ask for permission from your good office in acquiring required data for my study. The documents that I will be requesting are as follows: 1. Morbidity and Mortality Rates in Bicol Region concerning Kidney Diseases (last 5 years) 2. Leading or Top 3 Dialysis Centers in Legazpi City, Albay based on their capacity, patient statistics, and available services 3. List of all Dialysis Centers in Bicol Region including their address 4. Any current or proposed plans and programs related to kidney health in Legazpi City, Albay 5. Any information on infrastructure guidelines and facility standards for kidney specialized healthcare centers
