Hi, I would like to request the following data below, regarding the National Center for Mental Health (NCMH): Regarding the Site: 1) Architectural and Structural As-Built Plans of the NCMH 2) Site Lot No. and TCT No. of the NCMH 3) Copy of Land Title of the NCMH (for land area and lot bearings) 4) Vicinity Map of the NCMH 5) List of Existing Pavilions and Building in the Development (include the Patients it caters) General Information: 1) Patient Statistics (Inpatient/Outpatient per Psychiatric Service) 2) Staff Statistics (per Psychiatric Service and Others) 3) Organizational Structure of the NCMH (Overall and per Department/Psychiatric Service) 4) Psychiatric Service offered by the Institution ( 5) Hospital Operational Process and Maintenance Guidelines Others: 1) Relevant Law and Ordinances for Mental Health Facilities 2) Standard for Mental Health Facilities
