Good day! Can I request a copy of my parent's Vaxcert since they are not knowledgeable and familiar with making transactions with our modern technology like this. As their son, can I be the one who will request to get their vaccination certificate? Here are some of my details: Name: Leinard Sereban Trillana Birthdate: Nov. 12, 2003 Age: 21 years old Address: Brgy. Abo Nagcarlan, Laguna Below are the vaccination details of my parents for you to easily process my request: My father's vaccination details: Fullname: Agrifino Urriza Trillana Birthdate: Jun. 22, 1967 Contact number: 09352867270 Address: Brgy. Abo Nagcarlan Laguna 1st dose: Nov. 29, 2021 (Pfizer) 2nd dose: Dec. 20, 2021 (Pfizer) My mother's vaccination details are: Fullname: Rubylyn Sereban Trillana Birthdate: Feb. 16, 1974 Contact number: 09952365940 Address: Brgy. You have requested the Vaxcert request for my parent's

We understand your concern however, please be informed that the encoding of vaccination records is done by the concerned vaccination center. If you want, you may kindly coordinate with the Local Government Unit (LGU) where you received your vaccination and request for assistance to generate your vaccination certificate.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and [email protected] so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.

Your right to request a review

If you are dissatisfied with our response to your FOl request, you have the option to request an internal review. To initiate this process, kindly submit your written appeal for review addressed to the Central Appeals and Review Committee (CARC) at [email protected]. Please provide a detailed explanation of your dissatisfaction with our response within 15 calendar days from the date of receiving this response. We will conduct the review and communicate the outcome within 30 calendar days from the receipt of your review request.
