Pursuant to Executive Order No. 2 (s. 2016) on Freedom of Information and Presidential Decree 1586 (Philippine Environmental Impact Statement System), I respectfully request certified copies of all Environmental Impact Assessment-related records for the project entitled:"Construction of Shore Protection, Brgy. Libertad, Gen. Luna, Surigao del Norte (Part 2)" implemented by DPWH/LGU General Luna, with reported construction date around May 2023.Specifically, please provide: 1. Coverage screening report and classification (ECP, ECA, or non-coverage). 2. Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage (CNC) issued, with annexes and conditions. 3. Environmental Impact Statement (EIS), Initial Environmental Examination (IEE), or EPRMP, whichever applied. 4. Scoping documents, Terms of Reference, and any Public Hearing/Consultation records (notices, minutes, attendance sheets, photos, and any public-hearing waiver). 5. Self-Monitoring Reports (SMRs) and Compliance Monitoring Reports (CMRs) from 2023 to 2025, including inspection reports by EMB, post-construction reports on coastal impacts. 6. Any complaints, communications, or findings regarding impacts to adjacent properties or erosion. I prefer to receive copies by email . [email protected] If no such records exist, kindly issue a Certification of Non-Existence/Unavailability.This request is made under the DPWH Social and Environmental Management System Manual (SEMS) and DENR DAO 2003-30, both of which require EIA documentation and public consultation for coastal infrastructure.
