About my letter dated 12 August 2024, regarding your request made under Executive Order No. 2, s. 2016 on Freedom of Information in the Executive Branch; specifically your request on executive .
We wish to inform you that based on Freedom of Information - Standard Operating Procedure (FOI-SOP) in reference to FOI Agency Manual[1], the following are standard requirements in the processing of your FOI requests:
1. FOI Request (Form attached to the email) must be in writing and duly signed by the requester;
2. Must state the name and contract information of the requesting party;
3. Must reasonably describe the information requested, and the reason for or purpose of the request for information; and
4. A copy of duly recognized government issued Identification Card with photo.
We wish to inform you further, that upon review and assessment of the submitted documents, it was determined that Item Nos. 3 & 4 of the requirements were not met. To facilitate your request, please fill out the attached FOI Request Form and return it to us, along with a copy of your duly recognized government-issued ID, for immediate processing.
Moreover, while waiting for your reply, your request will be put on hold. If we do not receive a reply after sixty (60) days, your request will be deemed closed.
