According to PNP People's Freedom of Information (FOI) Manual, January 2021. Chapter 2 Procedure in Filing and Processing of the Request, 2.1 How request is made (a) Any person who requests access to information shall submit a written request to the concerned office/unit using the prescribed PNP FOI Request Form. Said form is widely available at various offices/units. It can also be downloaded from the PNP official website www.pnp.gov.ph which contains the link to the Freedom of Information Official Website https://www.foi.gov.ph that can be filled-out electronically by the requesting person. I was able to fill out the request form electronically in the FOI Official Website, why am I required to fill-out again the manual request form?

For proper documentation and endorsement to our Director, we require all requesting parties within our Unit to accomplish the attached FOI Request Form (manual copy) and provide additional information such as your office or agency affiliation. This serves as our internal reference and ensures that all requests are duly recorded and acknowledged by our Office Head prior to processing.
