Dear Sir/Maam,

I am wrinting in behalf of my Uncle, Alfredo Almo, with SSS number 0003-3097772, to express our concern and seek clarification regarding his current monthly pension.

My uncle has served as a seaferer for over 20 years, contributing regularly and substancially to the SSS throughout his entire career. Given his long tenure and the relatively high contributions made during those years, we were disheartened to learn his current monthly pension is only around Php 2,500.00.

We respectfully request a review and detailed explanation of how his monthly pension was computed. Specifically, we would like to understand:
1. The basis and formula used to calculate his pension
2. His total number of contributions and average monthly salary credit (AMSC)
3. Whether all his contribution, including those made through manning agencies or shipping companies, were properly recorded and posted.
4. If there are any issues or missing contributions affecting his pension amount.

We believe there may be a descrepancies or unposted contributions, and we would greately appreciate if your office could invistigate this matter. 
My uncle dedicated much of his life to working at sea and deserves to receive a fair and accurate pension reflective of his years of hardwork and contributions.

We hope for your immediate attention to this matter.

Your right to request a review
