Hi! I would like to request for a copy of the original birth certificate for my late grandmother’s sss death claim. She was born on January 16, 1937 but her records showed January 16, 1935 as she tampered the date to be able to work early. We are having a hard time submitting the SSS death claim as they are looking for the January 16, 1937. Please help! Or just kindly direct me to the right government agency to request from. Thank you!
