Dear Sir/ Madam,
I wouldl ike to request for the copy of decision from Hon. Renato D. Muñez, former presiding judge of Regional Trial Court of Cadiz - Branch 60, Negros Occidental rendered on November 05, 2007 under Civil Case No. 491-C for cancellation of Real Estate Mortgage w/ Damages, entitled "Sps. Romuel & Helen Dela Cruz vs. Rural Bank of La Carlota. Since RTC -Cadiz Branch 60 will not issue to us the said copy.
We will need it for the cancellation of notice lis pendens annotated on our Certificate of Title.

Dear Sir Edsel,
Thank you for the response. But I have already requested the said copy with Regional Trial Court (RTC) Cadiz - Branch 60. And according to the Clerk of Court VI of the said court, the said record can no longer be found.
The purpose of this record (decision) is for us to process the cancellation of notice of lis pendens annotated in the title of Atty. Andrea Z. Uy, et. al.
Also please see attached certification issued by Atty. Felix G. Guarnes, Jr.
