I refer to my letter of Apr 21, 2022 01:06:34 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request personal copy.

In order for us to work further on your request and to properly endorse your concern at the Revenue District Office (RDO) having jurisdiction over the place of your employer for 2015-2016 may we respectfully request for the following details:

> Name of your Employer and their Business Address; and
> Employer's TIN Number (if applicable), then email at sherry.ann.arcita@bir.gov.ph - since it contains sensitive information and indicate in your email your tracking number #BIR-816410603881, to respond and easy reference.

For BIR Form 1905, the form is already available at the BIR Website (www.bir.gov.ph) under Quick Links.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jul 15, 2022 02:57:48 AM, your request will be closed.
