We are requesting from your esteemed office a letter certifying that the following international air carriers of Philippine and foreign registry, as listed in Annex “A,” attached herein, have been issued or not issued the following permits:

1.	Foreign Air Carrier’s Permit;
2.	Permit to Operate International Air Transportation Services, whether scheduled or non-scheduled; 
3.	Permit to Operate Domestic Air Transportation Services, whether scheduled or non-scheduled; and/or
4.	Other relevant permits (charter flights);

While our aim is to provide information whenever possible, in this instance we would need for you to resubmit your request as we are still missing from you some vital information. Please ensure that, in addition to all the mandatory fields, you also indicate the names of the international air carriers which you seek information about. It appears that there is no such attachment, as indicated in your letter.

Your right to request a review
