I would like to raise a concern regarding my vaccination record. I lost my vaccination card last December. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in via Robinson Antipolo conducted under Reliance United at Antipolo City. My first dose was on October 4, 2021 and the second dose was at December 7, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

Is this request offensive?
