Hi! 

I would like to request information about my VaxCert application Reference ID: 1116337 & 1116459 originally applied for back in November 18, 2021 but have not gotten any help till this day. 

Its been more than 6 months ago but saw that they haven't updated or uploaded my information at all despite numerous follow-ups. 

I requested an update of information again last June 14, 2022, and got new reference numbers: 1425127 & 3710371 

I have uploaded supported documents but have not heard from them yet.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
