Hi! This request is on behalf of my daughter, ANGELI MIKAELA C. ALFAFARA, a minor during her 1st dose @ Mandaue City Sports & Cultural Center on Dec. 2, 2021. Her 2nd dose was @ Parkmall-Cebu on Jan. 13, 2022 (accompanied by her older brother). We secured an Affidavit of Loss for her lost vax card for her booster shot now that she's 18. However, both Parkmall & VOC-Mandaue could not find her 2nd dose record. (They even insisted she must have brought it home by mistake which she DID NOT.) Is there any way this can be resolved? Any help is greatly appreciated.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
