Good day!

I am writing this letter to request for a date change of my booster vaccine.  My first request here was granted but didn’t know that the date was the problem. Is it possible to change my vaccination date from July 7, 2021 to DECEMBER 7, 2021(correct date).  I already called the the LGU for this and ask them to change but it’s been 2 days. My flight is in May 18, 2022 and I am hoping to get this fix ASAP. Thank you and God Bless!

Response to Request

Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
