Greetings! I am requesting another vaccination card and booster shot card. I do not have any pictures. However, I know the exact date when and where I got all my shots. My 1st dose was on the 6th of October of 2021, 2nd dose was on the 27th of October, same year. I received my booster shot on the 19th of February. I received them all at Batangas City Sports Center at A delas Alas Dr Rd, Batangas under the name Maui Shaneal D. Bautista. I have already looked up my records at VaxCertPH and they have my records. I hope my request is fulfilled as I will be using these for my face-to-face classes.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
