I would like to request for a replacement of my Covid-19 Immunization Record Card because it was lost. I do not have a copy but I can remember the following details:

 1st Dose - August 20,2021, Pfizer, Makati CIty
2nd Dose - September 10,2021, Pfizer, Makati City

Hoping you could assist me on this matter.

Your FOI request is approved.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
