Hi!

 I would like to request a Booster Certificate, for this is one of the requirements for my studying abroad. NMMC just have me a sticker and below are the information. But this is not accepted by the school where I am going to study.

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may call the Vax Cert
Customer service Hotline. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
