To whom it may concern,
I would like to change my birthmonth on my vaccine card. I've realized that i had written the wrong birthmonth when I was signing up for my 1st dose. Please change it from June 21, 2003 to April 21, 2003. I might run into some problems in the future with any of my school/government documents.

vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
