I would like to request the correction of my surname in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my surname is VELEZ, which supposed to be BARADILLO. I only had my VELEZ ID available when I took my vaccine and I’m still correcting all my govt records to BARADILLO.

You asked for To correct my surname in the Vaxxcert.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
