You asked for COVID vaccine record or booster card.

Response to Request

Your FOI request is approved. 

Please be advised that COVID vaccination card is provided by the LGU. If you lost /misplaced your vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.

Your right to request a review
