Hi!
I am Jefrey Delos santos and I need a vaxcert for my travel to Boracay in the following week. The time that I registered online it appears only the first dose, second dose has no record even I got 2 doses already. My first dose was on nov.11,2021 and my second dose was on December 16, 2021. I have my proof which is my LOCAL LGU COVID-19 VACCINATION CARD. I got the 2doses of vaccine in ALAMINOS,LAGUNA PHILIPPINES.

Please update it coz its been 6 months' already and i badly need it for our travel. You can send it to my email: delossantosjefrey0@gmail.com

Your FOI request is approved.
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
