Hi, my first Booster Moderna taken in Sucat Paranaque has not reflected my record on Vax Cert PH only my first and 2nd dose of Aztra Zeneca conducted in City of Muntinlupa. I already have send my letter of request for updating record of my 1st booster shot with reference number 1405984 with uploaded supporting documents (Proof of Booster Vaccination with Name of the person Initiated the Vaccine and stamped date), ID and Airline Ticket to expedite the request. To date, no update yet after 3 emails to vaxcertsupport@doh.gov.ph and a communication to one of their Customer Service. Please help.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
