Hello!

I would like to request correction of my birthdate and missing record of 2nd dose vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my birthdate was August 7, 1995 was suppose to be July 8, 1995.
And I found out that on the DOH vaxcert my 2nd dose was not on record.
Below are all the details.

You asked for For Correction of Vaccination Record (Incorrect Birthdate and Missing 2nd Dose Record).

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
