Hi. I would like to request my daughter's middle name be corrected. The middle name on the record for the vax cert is Centos whereas it should be CANTOS.
My daughter's name is GILLIAN CANTOS DELA CRUZ

You asked for Vaxx Cert.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
