Hi! I would like to request to change my son's name on his vaccine certificate. I just recently acquired his PSA Birth Certificate and I learned that we have been using wrong details of his name. I am enrolling him at a university and I would like his PSA Birth certificate and his Vax Certificate to match as it is one of the school requirements for his admission.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.
Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
