Hi - I would like to follow-up the status of my open tickets in VaxCert PH. The previous one got rejected and i have not heard any update since then. Japan Fast track website requires 3 doses. I am missing my booster dose.

#3879298 - Rejected due to duplicate request (I have not request anything prior)

#1875365 - Open

Also, the contact # 88-7614-88 provided in the website can't be reached.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
