Good day! 

I lost my vaccine card last June 17, 2022. I would like to request for a new one. 

I had my primary vaccines in San Mateo Rizal but the two other boosters in Marikina City.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
