Hello, I would like to request for vaccination certificate for my uncle (Ignacio C. Martinez Jr.) He had his booster shot done here in Lucena City, Quezon, Philippines last Dec. 22,2021 (lot number: 32025BD) given with a piece of paper as proof. He then flew back to London. Last June 28, he was due to go back to the Philippines but he was denied to board the plane as his booster shot record was not honored. Kindly help to give legit proof of his booster shot as it was not found on vax cert PH website.

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
center.
For further assistance, you may call the Vax Cert
Customer service Hotline. Here are the contact

Kindly coordinate also with the LGU where you
received your vaccination and request for
assistance to generate your vaccination
