Hi Good Morning. 
I am Carlos Trinidad Jr., father of Carl Zedrick D. Trinidad would like to request your good office for a copy of my son's Covid 19 Vaccine Card. Because it was misplaced and lost on our way to province. I wasnt able to get the certifucate on Vaxcertph because of the details i cant provide. like date of vaccinations.
We badly needed a copy for my son's School requirement on Transferring procedure.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
