Hi! I would like to follow up my Vax Cert Request with: 

1. Ticket Number: 804729-1

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last year, Dec 01, 2021. 

However, your system always declines it and always tells me that there is no vaccination record found. I have already submitted the copy of my vaccination card and my Philhealth ID in your system, with a ticket number of: 804729-1

I am really hoping you can help me.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.

For further assistance, you may call the Vax Cert Customer service Hotline. Here are the contact details:

Kindly coordinate also with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you.
