Hi! I would like to request to correct the surname in my Covid-19 Vaccination card. The name listed is Joanna Marie Arambulo (my husband’s surname) since they copied it from my medical records. But my present name is my maiden name , Joanna Marie Javier. I will be needing the correct card for my vaxcert for travel purposes because in my passport use my maiden name.

Response to Request

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
