Hi! I lost my vaccine card and I don't have any copy nor picture, but I do remember when and where I was vaccinated. Is there any way for me to get a new one so I could get my booster shots?

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
