The DOH Vax Certificate does not reflect the Moderna booster shot I got from the SIVC NPF Paranaque on JUL 11, 2022, batch no. 2100694.
My DOH VAX Cert only reflects 3 Doses, should be 4 doses. I have 2 full doses of Pfizer and 2 Moderna as Booster

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
