I would like to raise a concern regarding my vaccination record. I lost my vaccination card last August. I'm fully vaccinated but when I checked my record through, it says that they have no record of my vaccination. 

I was vaccinated in SM SAN MATEO Rizal . My first dose was on October 11, 2021 and the second dose was at November 03, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

Your FOI request is approved. 

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. If your vaccination record is
unavailable, the record may not have been
uploaded yet in the Vaccination Information
Management System (VIMS) by the vaccination
About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
