Hello! I would like to request for information regarding:

1. Total Number of patients with Chronic Kidney Disease in San Jose and Lipa City, Batangas for the past 10 years. 
2. Total Number of patients with kidney cysts in San Jose and Lipa City, Batangas for the past 10 years. 
3. Total Number of patients with kidney inflammation in San Jose and Lipa City, Batangas for the past 10 years.

NOTE: Please INCLUDE patients who are not receiving dialysis treatment but diagnosed with these diseases. It's okay if the data is only until 2015. Thank you :)

You asked for data on the Number of CKD patients in San Jose and Lipa City, Batangas as attached.
