In order for us to work further on your request, we need more information from you regarding your query. Based on available records in our College you have not graduated Grade 12 here at Bulacan Agricultural State College. In relation to this, may we know where you graduated your Grade 12.

My apologies to whom it was concerned, I graduated grade 12 in Montessori De San Ildefonso located in San Ildefonso, Bulacan. As of now I am now clarified to get Form 138 instead in my senior high school. Thank you for the effort I really appreciate it.

Information you requested is not being maintained by the College. And this can be sourced from the Senior High School you graduated from.
Your right to request a review
