Hi,
Gusto ko sanang ifollow up yung request kong DOLE Certificate para sa pagclaim ko ng Unemployment Benefit. Isang buwan mahigit na yung request ko pero hanggang ngayong wala akong narereceive mula sa DOLE. Sana naman ay mapansin nyo yung request ko kasi isang buwan ng mahigit ito. Salamat

Your Request

You asked for DOLE Certification for SSS unemployment benefit.

Response to Request

This is to inform you of the process and requirements by which to apply for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION for SSS issued by DOLE-NCR.  An employee - Local or Overseas Filipino Worker (OFW) who was involuntarily separated by their employer may file their application for the issuance of CERTIFICATE OF INVOLUNTARY SEPARATION at the DOLE-NCR website thru the e-filing services tab by clicking this link: https://docs.google.com/forms/d/e/1FAIpQLSfhNqxP-qEiWAZUmjuTK8ayWHRxFcEfZeJn3kLmv2zDSrAiYw/viewform
The requirements for the issuance of the certificate of involuntary separation for uploading online are as follows: 
a.	valid email address (google account)
b.	at least one (1) valid I.D. (scanned copy)
c.	scanned copy of the Notice of Termination issued by the employer, in the absence of Notice of Termination, an applicant may submit a duly notarized Affidavit of Termination of Employment 
Please note of the Process Forms of the E-filing services for Certification of Involuntary Separation for SSS:
Step 1: Fill-out google forms
Step 2: Submit form/s and other requirements
Step 3: Receive email acknowledgement receipt of DOLE-NCR
Step 4: Get notified by DOLE-NCR on the updates and result of application and/or request

Your right to request a review
