Good day. I am April V. Rivera, teacher from Angono, Rizal. I would like to follow up my request regarding the update of my record of paid premiums. I've been an active member since December 2009, but there are lapses in my record of paid premiums. I already submitted thru GSIS NCR email the following requirements: 
1. MRF dated June 30, 2021
2. 2 IDs
3. Remittance list with OR from the agency

Until now, GSIS NCR has no response regarding this issue.
