    How was your request?

Pagibig Contribution remitted by my employer

Requested from HDMF by M. F at 03:08 PM on Aug 25, 2021.
Purpose: To validate the contribution from my previous employer and file case if needed
Date of Coverage: 04/01/2002 - 08/25/2007
Tracking no: #HDMF-813148625637

F 03:08 PM, Aug 25, 2021

I was looking into my records for the contribution. I do understand that SSS, Philhealth and Pag-ibig are mandated and automatically deducted from my pay. However, upon checking my pag-ibig contribution and Philhealth, it seems it doesn't tally with my SSS Contribution based on days I was employed. I am particular to the missing Pag-ibig contribution where I was employed under the company that was acquired and renamed to several name " ContactWorld > ServiceZone > ClientLogic > now SITEL " The Country manager at that time of my exit (year 2006) was Danilo Sebastian Reyes / Dan Reyes.
