Hello. This is regarding the Endowment Fund Program of PCSO. One of the requirements for hospitals to be able to avail the said program is the Hospital Budgetary Requirement. Hence, may I request for a form/format or sample of the said requirement? We will be using this for submission of the above program.

Your FOI request is approved. Please find attached herein Application Form for Endowment Fund and the checklist of requirements and submit the same to the PCSO Charity Assistance Department, 3/F Conservatory Building, 605 Shaw Boulevard, Mandaluyong City.



Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Office of the General Manager at 4th Floor Sun Plaza Building, Shaw Blvd., Mandaluyong City.
