Good Day Dept Ed,

I'm requesting to have my daughter change her last name. She's an incoming Grade V and having difficulty enrolling her since someone submitted an invalid birth cerficate when she was in Grade 1. I already got the PSA original Birth Certificate but the school stated I should back to her old school to have it corrected. 

Please help me to have this resolved.

You asked for Request of Last Name Change in School record.

Response to Request

Please be informed that the inconsistency of a person's records is not only an issue at the Department of Education (DepEd) but a legal issue as well.

Both records should be reconciled and should be decided as to what name to follow.

Kindly secure and submit the following pertinent documents to the Legal Unit of the nearest DepEd Regional Office:

-Original Birth Certificate
-Joint Affidavit of Two (2) Disinterested Persons
-Affidavit of Discrepancy (if of legal age)
-Mother's/Father's Affidavit (if a minor)
-Request letter addressed to the DepEd Regional Director specifying the intent to change the name from (WRONG NAME/S) to (CORRECT NAME/S)
-Form 137 (Original or Certified True Copy)
